Original language: PolishPrzedmiotem zamówienia są dostawy płynów infuzyjnych oraz produktów do żywienia pozajelitowego, dojelitowego i doustnego, według poniższej części: Zadanie 11. Szczegółowy opis przedmiotu zamówienia ww. części został wykazany i opisany w „Formularzu cenowym” (załącznik nr 2 do SWZ) oraz we wzorze umowy (załącznik nr 3 do SWZ) stanowiącym integralną część Specyfikacji Warunków Zamówienia. Warunki umowne realizacji przedmiotowego zamówienia publicznego zawiera wzór umowy, stanowiący integralną część SWZ. W przypadku zaistnienia podstaw do powierzenia przetwarzania danych osobowych Wykonawca zobowiązuje się do podpisania umowy powierzenia przetwarzania danych osobowych (zgodnie z załącznikiem nr 11 do SWZ). Okres obowiązywania umowy wynosi 24 miesiące licząc od dnia zawarcia umowy, zgodnie z § 7 wzoru umowy. Termin realizacji przedmiotu zamówienia, zgodnie § 3 wzoru umowy. Podana szacunkowa wartość bez VAT jest całkowitą wartością przedmiotu zamówienia z „prawem opcji”.
