Purchase of PET/CT and SPECT devices for Copernicus PL Sp. z o.o.

1. The subject of the order is the purchase of PET/CT and SPECT devices for Copernicus PL Sp. z o.o. The subject of the order consists of 1 part, which is divided into 2 tasks: 1) Task No. 1 – PET/CT device (1 pc), 2) Task No. 2 – SPECT device (1 pc), 2. The detailed scope of the subject of the order, the list of devices, quantities and the method of order execution are included in the annexes to the SWZ, namely: 1) Annex No. 1A to SWZ – Price form, 2) Annex No. 1B to SWZ – Description of the Subject of the Order, 3) Annex No. 3 to SWZ – Draft agreement. The Contractor attaches Annex No. 1A and 1B to SWZ to the offer. By filling in the indicated positions in Annex 1B to SWZ- OPZ, the Contractor confirms compliance with the requirements specified by the Contracting Authority in tasks no. 1 and 2. 3. The offered subjects of the order must be complete, of high standard in terms of quality and functionality, and free from material, structural and legal defects. 4. The offered subject of the order must be approved for circulation and use in accordance with applicable legal regulations. 5. The subject of the order must comply with the Act of April 7, 2022 on Medical Devices (Journal of Laws 2022.974 of May 9, 2022). 6. In all provisions of the SWZ and its annexes, where trademarks, parameters, sources or a specific process characterizing products supplied by a specific contractor of origin (manufacturer or device names) are indicated, in accordance with art. 99 sec. 5 and when the subject of the order is described by reference to standards, technical assessments, technical specifications, reference systems, in accordance with art. 101 sec. 4 of the Pzp Act, the Contracting Authority allows equivalent solutions. In such a case, the Contractor is obliged to prove in the offer, in particular by means of subject-matter evidence, that the proposed solutions meet the requirements specified in the description of the subject of the order to an equivalent extent. 7. The Contracting Authority does not reserve the possibility of applying for the order only by Contractors referred to in art. 94 of the Pzp Act. 8. The Contracting Authority does not impose the requirements referred to in art. 96 sec. 2 pt. 2 of the Pzp Act. 9. Source of financing for the public contract: The order concerning tasks 1 and 2 is financed from a project under the National Reconstruction and Resilience Plan: Obtaining access to the highest quality oncological therapy and diagnostics through the purchase of equipment for COPERNICUS Medical Entity Sp. z o.o. in Gdańsk. Component D “Efficiency, accessibility and quality of the healthcare system”. Investment D1.1.1. “Development and modernization of the infrastructure of highly specialized care centers and other medical entities”. The amount of VAT is not an eligible expense within the aforementioned project and the Contracting Authority will finance it from its own funds. 10. Due to the financing of the purchase from the aforementioned project, it is required that the purchased assortment complies with the principle of "do no significant harm to the environment" (DNSH).
