Tender inquiry for comprehensive design and execution of construction works in the "design and build" formula within the project entitled "Expansion and equipping of NZOZ Sokrates in Zgłobień – Mental Health Center for Children and Youth with a day ward, II reference level".

1. The subject of the order is the comprehensive design and execution of construction works in the "design and build" formula within the task entitled "Carrying out construction works for the expansion of the Mental Health Center in the scope of the children's ward and outpatient clinic", implemented within the framework of project no. FENX.06.01-IP.03-0081/25 entitled "Expansion and equipping of NZOZ Sokrates in Zgłobień – Mental Health Center for Children and Youth with a day ward, II reference level". 2. The investment will be carried out in the existing facility of the Non-Public Health Care Facility Sokrates in Zgłobień and on the adjacent land, at the address: Zgłobień 67A, 36-040 Boguchwała, on plots no. 1179, 1180/1 and 1181, cadastral district 0010 Zgłobień. 3. The subject of the order includes, in particular: 1) carrying out necessary surveys, measurements, tests, excavations, analyses and expert opinions necessary for the proper design and execution of the investment; 2) development, based on the Functional and Utility Program, of complete multi-branch design documentation, including all elaborations required by law and the Functional and Utility Program, including documentation for interior design and equipment as required by the Functional and Utility Program, as well as solutions necessary for the execution of construction and installation works and preparation of infrastructure for equipment purchased in separate proceedings; 3) obtaining, on behalf of the Contracting Authority, based on the granted power of attorney, all required technical conditions, opinions, approvals, consents, decisions and administrative decisions, including the final and legally binding building permit decision; 4) supervision by designers during the execution of construction works; 5) execution, based on the approved design documentation, of all construction, installation, assembly, finishing and related works, land development, and execution of required installations, networks, connections and technical devices, as specified in the Functional and Utility Program; 6) supply and installation of materials, construction products, fittings, installations and technical devices that are part of the building or necessary for its proper functioning, excluding equipment referred to in Section 3, point 8; 7) execution of required tests, examinations, checks, measurements, adjustments, start-ups and technical acceptances, preparation of complete as-built documentation and obtaining, on behalf of the Contracting Authority and based on the granted power of attorney, the final and legally binding permit for use of the facility, including preparation and compilation of required documentation, making required notifications, submitting applications, participation in mandatory inspections and removal of any deficiencies and irregularities identified by the competent authorities; 8) handover to the Contracting Authority of a complete facility, ready for use – within the scope of this order – corresponding to the function of the Mental Health Center for Children and Youth with a day ward of the II reference level. 4. The detailed scope of the subject of the order and the minimum technical, functional, quality and utility requirements are specified in the Functional and Utility Program along with attachments, which constitutes Annex No. 7 to the tender inquiry. The Functional and Utility Program, clarifications provided by the Contracting Authority, and changes to the tender documentation made before the deadline for submitting offers, are an integral part of the order conditions. 5. Auxiliary documentation, which is Annex No. 8 to the tender inquiry, is for informational and auxiliary purposes only. The Contractor is obliged to verify it for suitability and compliance with the Functional and Utility Program, order conditions, factual status and applicable regulations. This documentation may be used by the Contractor only to the extent that it is consistent with the above requirements. Its provision does not release the Contractor from the obligation to independently develop complete and correct Design Documentation, nor does it limit their liability for its content, completeness and compliance with the Agreement. 6. The Contractor is obliged to execute the subject of the order in accordance with the Functional and Utility Program, approved design documentation, building permit decision, contract provisions, applicable regulations, principles of technical knowledge, and requirements for facilities where medical activities are carried out. The design documentation and executed works must take into account the principles of universal design, accessibility requirements for persons with special needs, the "do no significant harm" (DNSH) principle, and environmental protection requirements relevant to the implemented Project. 7. Works will be carried out in the facility where health services will be provided during the execution of the order. The Contractor is obliged to organize the works in a way that ensures the safety of patients and staff and the continuity of health services. The Contracting Authority does not permit the temporary closure of the entire building. Temporary closure of individual rooms, installations or parts of the facility is permitted only after prior agreement with the Contracting Authority, under the terms specified in the draft contract. Closures should be limited to the necessary minimum and, if the nature of the work allows, carried out outside of health service hours. During the hours of 7:00 AM to 8:00 PM, a single interruption in the water or electricity supply to the part of the facility affected by the closure cannot last longer than 1 hour. The deadlines and duration of closures outside the indicated hours require prior agreement with the Contracting Authority in each case. 8. The scope of the order does not include the supply or installation of medical and non-medical equipment, which will be purchased in separate proceedings, in particular: medical and non-medical equipment...
