Supporting People at Home: Ongoing Care and Reablement
Description
The London Borough of Camden is seeking to award contracts for care and support at home services across two connected areas: Ongoing Care and Reablement. For the purposes of this procurement exercise, the Services have been divided into two Lots. Lot 1 (Ongoing Care) is further divided into five (5) sub-lots, and Lot 2 (Reablement) is further divided into three (3) sub-lots, as outlined below.
Ongoing Care sub-lots:
Lot 1a West
Lot 1b North
Lot 1c East
Lot 1d Central
Lot 1e South
Reablement sub-lots:
Lot 2a North West locality
Lot 2b North East locality
Lot 2c South locality
The Services for this integrated model will provide:
โข Support to maintain or prevent deterioration in health and wellbeing which reduces the need for unnecessary hospital or residential care admission;
โข Ongoing Care with personal care, physical health care and support with daily living which the person cannot complete without assistance; or
โข Intensive support following a period of hospitalisation or treatment, which enables the person to regain confidence and improve self-care in their own home and engage with their local community.
The delivery of approximately 248,000 hours of ongoing care in the West of Camden annually.
The delivery of approximately 131,200 hours of ongoing care in the West of Camden annually.
The delivery of approximately 306,900 hours of ongoing care in the East of Camden annually.
The delivery of approximately 220,600 hours of ongoing care in the Central of Camden annually
The delivery of approximately 166,300 hours of ongoing care in the South of Camden annually.
