Als Casemanager Jeugd zorg jij ervoor dat jeugdigen en gezinnen die moeilijkheden ervaren de juiste begeleiding krijgen. Je bent verantwoordelijk voor het analyseren van de situatie en de vraagverheldering van het gezin om tot een passende oplossing te komen. Het is van belang om de veiligheid in kaart te brengen en te anticiperen op signalen van onveiligheid binnen het gezin. Je betrekt ouders en het sociale netwerk bij het zoeken naar een passende oplossing en werkt samen met diverse partners zoals school, politie, zorgaanbieders, jongerenwerkers van Sociaal Werk Oldambt en collega?s binnen de gemeente. Als casemanager zorgtoewijzingen ben je niet langdurig betrokken bij het gezin maar maak je een analyse en uiteindelijk een advies met betrekking tot de in te zetten ondersteuning. Jouw werkplek Het Jeugdteam in OIdambt is een goed functionerend team, waar de processen op orde zijn en steeds de vernieuwing, verbetering en samenwerking met anderen worden opgezocht. De sfeer is goed en we heten je van harte welkom! Je doet het niet alleen Binnen het Jeugdteam krijg je ondersteuning van een gedragswetenschapper, een praktijkbegeleider en een inhoudelijk co?rdinator en bespreek je casussen in casu?stiek.
