Към техническото предложение по всички обособени позиции- ЗА ДОКАЗВАНЕ на съответните характеристики се прилагат: каталози на фирмите производители с каталожни номера и/или други документи (ръководства, инструкции за употреба на производителя, извадки от интернет страница на производителя и други, с каталожни номера) от които е видно, че оферирания продукт отговаря на изискванията на Възложителя заложени в техническата спецификация. Към техническото предложение ПО ВСИЧКИ ОБОСОБЕНИ ПОЗИЦИИ се ПРИЛАГА и копие от Декларация за съответствие с Директива 98/79/ЕС /за диагностичните реактиви, представляващи ин витро диагностични медицински изделия (или еквивалент) или копие от Декларация за съответствие с Регламент (ЕС) 2017/746 на Европейския Парламент и на Съвета за медицинските изделия за инвитро диагностика. Съгласно изискванията на чл. 37, ал.1 от НАРЕДБА № 7 за условията и реда за съставяне на списък на медицинските изделия по чл. 30а от Закона за медицинските изделия и за определяне на стойността, до която те се заплащат, участникът следва да ДЕКЛАРИРА, че: оферираните медицински изделия отговарят на изискванията на Закона за медицинските изделия; оферираните медицински изделия са включени в списъка по чл.1 т.1 буква “б“ от Наредбата няма регистрирани данни в ИАЛ и/или ЕUDAMED за инциденти/сериозни инциденти през последните 18 месеца, както и за блокирани или изтеглени от пазара партиди по причини, свързани с безопасността на медицинските изделия през последните 18 месеца съгласно предоставена от ИАЛ информация; същите се заплащат в поне три от държавите - членки на Европейския съюз, чрез съответните им системи за заплащане с публични средства или съответните им здравноосигурителни фондове. За обособените позиции, означени като комплексни /с подпозиции/, участниците трябва да подадат оферта за всички подпозиции, обхванати от съответната обособена позиция, в противен случай се отстранява от процедурата.
