Door middel van het doen van onderzoek en het opstellen van een ondersteuningsplan analyseer en indiceer je de zorg(-behoefte) van de zorgvrager.Als consulent(e) Team Jeugd werk je specialistisch. We werken integraal, wijkgericht en vinden de vraag achter de vraag enorm belangrijk.Waar mogelijk organiseer jij kortdurende ambulante trajecten waarbij je oplossingsgericht werkt. Uiteraard heb jij ook aandacht voor:Het signaleren van de ondersteuningsvraag en het ondersteunen bij het vinden van een oplossing binnen het (wijk)netwerk (persoonlijk en professioneel);Het activeren van de inwoner in het ontdekken van eigen mogelijkheden; Het samen met de inwoner opstellen van het ondersteuningsplan en het administratief verwerken van dat plan; Regievoering op de uitgezette zorg (intensieve) procesregie.Verder:Word je ingezet in crisissituaties waarbij je oplossingsgericht de crisis aanpakt;Heb je oog voor- en maak je een risico-inschatting met betrekking tot veiligheid binnen het gezin;Je bent in staat om de vraag van de inwoner te vertalen in een gericht advies dan wel het in gang zetten van een aanvraagprocedure zorg;Je kunt communiceren op diverse niveaus, zowel binnen de gemeente, als met andere samenwerkingspartners;Je kunt goed omgaan met problematiek die zich voordoet bij zorg/crisissituaties.Concreet wordt er in deze opdracht het volgende van je verwacht:Je draait een caseload en voert procesregie. Je bent bekend met de grenzen van de jeugdwet. Je bent in staat om te normaliseren en demedicaliseren.Na gunning van de opdracht dient de gegunde partij een recente VOG aan te bieden. Na zorgvuldige afweging achten wij deze opdracht niet geschikt voor een zelfstandige/zzp-er. Wij willen zelfstandigen vragen om zich te laten inschrijven bij een bureau ?n zich in loondienst te laten voorstellen op deze opdracht, mochten zij hiervoor in aanmerking willen komen.
