入札は消費税等額を除いた督促状、納付書、通知文書、独自通知文書、窓開き封筒のそれぞれの印書並びに作業等に係る各単価と、当該単価にそれに対応する上記件数を乗じて得た概算金額にて行い、落札決定に当たっては、入札書に記載された金額をもって落札価格とする。
２競争参加資格
(1) 令和07･08･09年度厚生労働省競争参加資格（全省庁統一資格）において、「役務の提供等」で「Ａ」、「Ｂ」の等級に格付されている者であること。
(2) 予算決算及び会計令（以下「予決令」という。）第70条の規定に該当しない者であること。
なお、未成年者、被保佐人又は被補助人であって契約締結のために必要な同意を得ている者は、同条中、特別の理由がある場合に該当する。
(3) 予決令第71条の規定に該当しない者であること。
（4) 資格審査申請書又は添付書類に虚偽の事実を記載していないと認められる者であること。
(5) 経営の状況又は信用度が極度に悪化していないと認められる者であること。
(6) 次の各号に掲げる制度が適用される者にあっては、この入札の入札書提出期限の直近２年間（[5]及び[6]については２保険年度）の保険料について滞納がないこと。
[1] 厚生年金保険
[2] 健康保険（全国健康保険協会が管掌するもの）
[3] 船員保険
[4] 国民年金
[5] 労働者災害補償保険
[6] 雇用保険
(7) 厚生労働省から指名停止の措置を受けている期間中の者でないこと。
(8) 当該業務を確実に実施できると認められる要員及び設備等を有している者であること。
なお、下記の書類により確認を行うため、入札参加書類と併せて提出すること。
・会社履歴書又はこれに類する書類
・設備の内容及び数量を示す書類（機械設備、機械台数、輸送設備、品質管理設備、人員等について記載したもの。既成のものがあればそれで可。）
(9) 品質管理体制について「ISO9001：2015」、組織としての能力成熟度について「CMMIレベル３以上」のうち、いずれかの認証を受けていること。
(10) 「情報セキュリティマネジメントシステム（国際基準規格ISO/IEC27001又は日本産業規格JISQ27001の認証」又は「プライバシーマーク（JISQ15001）」のうち、いずれかを取得していること。
(11) 過去に本事業と同等規模以上の類似業務の実績を有していること。
(12) 厚生労働省保有個人情報等管理規程に基づき、適切に個人情報の管理を行うこと。
(13) その他予算決算及び会計令第73条の規定に基づき、支出負担行為担当官が定める資格を有する者であること。

４　電子調達システムの利用
本案件は、電子調達システムで行う。
なお、電子調達システムによりがたい者は、書面により申し出た場合に限り、紙入札方式に代えることができる。紙入札方式の場合については郵送にて厚生労働省競争参加資格（全省庁統一資格）及び入札書の提出を行う。

この入札に参加を希望する者は、調達物品の品質、特質及び条件に適合する競争参加資格に関する証明書等を入札説明書において示す日時までに提出しなければならない。競争参加希望者の関係書類は当省において審査するものとし、採用し得ると判断された者のみ入札に参加することができる。
なお、入札者は開札日の前日までの間において、支出負担行為担当官から当該書類等に関し説明を求められた場合は、それに応じなければならない。
また、入札に参加を希望する者は、支出負担行為担当官が指定する暴力団等に該当しない旨を記した誓約書を提出しなければならない。
(4)　留意事項
ア　入札者から提出される契約関係書類については、事業者としての決定であることを要する。
イ　契約関係書類に虚偽記載等の不正が発覚した場合は、契約解除は違約金を徴収することがある。
(5) 入札の無効
本公告に示した競争参加資格のない者の提出した入札書、入札者に求められる義務を履行しなかった者の提出した入札書その他入札の条件に違反した者の提出した入札書は無効とする。
また、(3)の誓約書を提出せず、又は虚偽の誓約をし、若しくは誓約書に反することとなったときは、当該者の入札を無効とする。

本公告に示した役務を履行できると支出負担行為担当官が判断した入札者であって、予算決算及び会計令第79条の規定に基づいて作成された予定価格の制限の範囲内で最低価格をもって有効な入札を行った者を落札者とする。ただし、落札者となるべき者の入札価格によっては、その者により当該契約の内容に適合した履行がなされないおそれがあると認められるとき、又はその者と契約を締結することが公正な取引の秩序を乱すこととなるおそれがあって著しく不適当であると認められるときは、予定価格の制限の範囲内の価格をもって入札した他の者のうち最低の価格をもって入札した者を落札者とすることがある。

[1] Have Grade A,B, on “Form print” among “Manufacture of product” in terms of qualification for participating in tenders by Ministry of Health, Labour and Welfare (Single qualification for every ministry and agency) in the fiscal years 2025,2026 and 2027
[2] Not come under Article 70 of the Cabinet Order concerning the Budget, Auditing and Accounting. Furthermore, minors, Person under Conservatorship or Person under Assistance that obtained the consent necessary for concluding a contract may be applicable under cases of special reasons within the said clause
[3] Not come under Article 71 of the Cabinet Order concerning the Budget, Auditing and Accounting
[4] Prove to have no false statement in tendering document
[5] Prove neither the business condition nor credibility is deteriorating.
[6] For bidders subject to the following insurance system of each item , and it is not in arrears either of insurance premiums (2 insurance year about (ⅴ) and (ⅵ)) for the last two years of the bid submission deadline of this bid：(ⅰ)　Welfare annuity insurance(ⅱ)　Health insurance (the thing which association of national health insurance has jurisdiction over)(ⅲ)　Seamen's insurance(ⅳ)　National pension(ⅴ)　Worker's compensation insurance(ⅵ)　Unemployment insurance
