In the offer, the Contractor must provide the name and address of the facility along with the business activity location number in BDO and the waste processing code (according to Annex 1 to the Waste Act of December 14, 2012), to which the waste will be subjected. The Contractor collecting waste with code 17 09 04 from the Contracting Authority is obliged to deliver it directly to the facility indicated in the offer. Waste with code 17 09 04 will consist, among other things, of various types of plastics, paper and cardboard, wood, metal, glass, rubble, and primarily various types of used building materials and interior furnishings removed during renovations. The Contracting Authority will allow the Contractor to take samples of the subject waste for their own quality parameter tests. The Contracting Authority also allows for a site visit and a trial collection of waste with code 17 09 04. The collection of waste with code 17 09 04 will take place from Monday to Friday between 6:00 AM and 6:00 PM, in previously agreed quantities (by e-mail) by the Contracting Authority with the Contractor's representative. Weighing of the waste collected by the Contractor will take place on the Contracting Authority's legalized scales and will be confirmed by a weighing slip and a waste transfer card generated in the BDO system. These documents will form the basis for determining the quantity of waste collected with code 17 09 04 and for issuing a VAT invoice. Settlement of the service performed by the Contractor based on the weight of the waste indicated on the waste transfer card accepted by the facility receiving the waste for management is permitted, only and exclusively with the written consent of the Contracting Authority.
